The personal information on this form is collected by the Local Government Investigations and Compliance Inspectorate (Inspectorate), a unit within the Department of Planning and Community Development (DPCD), for the purposes of distributing Inspectorate electronic newsletters and bulletins (Newsletters) to key stakeholders within the Victorian Government.
Personal information collected on this form may also be used to evaluate the effectiveness of the Newsletters.
Where you do not provide the information required by this form (for example your first name and email address) we may be unable to process your request.
Personal information will be disclosed to Adz@work who will be responsible for distributing the Newsletter and to Vision6 who manage the database which stores and processes the information on the forms completed for the Newsletters.
You can request access to your personal information by contacting the Inspectorate. If informal access cannot be given to you by that section, contact DPCD’s Freedom of Information unit by phone (03) 9208 3112 or email foi@dpcd.vic.gov.au.
